We are extremely pleased that you have chosen Larson Medical Aesthetics for your aesthetic needs. This information regarding financial matters will be helpful to you in understanding our charges. All patients must accept this FINANCIAL POLICY before receiving treatment.

We accept the following forms of payment: CASH, CHECKS, VISA, MASTERCARD, DISCOVER and AMERICAN EXPRESS. We accept the above credit cards. If charges are disputed or refunds requested you wave your right to HIPPA privacy practices.

*PLEASE NOTE: Checks exceeding $500.00 WILL NOT be accepted.

All sales of product/services are final. No refunds will be granted.

A $40.00 service charge will be assessed for returned checks.

We collect a credit card number to secure your appointment at time of scheduling, a $100 deposit is required to secure that appointment, we will hold the card number on file.

A $100.00 charge will be assessed for rescheduled, cancelled no-show appointments if we do not receive notification within 48 hours of the scheduled appointment. The credit card we have on file will be automatically charged.

I understand that if I cannot make an appointment with the 10-minute grace period I must call 206-244-5477 to reschedule my appointment.
